Advanced Pharmacist - Cardiology

Job summary

An exciting opportunity has arisen for a motivated and enthusiastic clinical pharmacist with a keen interest in cardiology to join our integrated pharmacy team. This post will play a key role in leading and developing cardiology pharmacy services across the Central Integrated Health Community (IHC).

The Central IHC is home to the North Wales Cardiac Centre based at Glan Clwyd Hospital, and the successful candidate will work closely with the cardiology multidisciplinary team across the coronary care unit, cardiac catheter laboratories and cardiology wards.

This role includes a defined commitment working with the Heart Failure team, supporting outpatient clinics, inpatient ward rounds and multidisciplinary team working to optimise evidence-based therapy for patients with heart failure.

The pharmacy team is closely integrated within the Central IHC, with opportunities to work collaboratively with cardiology pharmacists across other localities, contributing to guideline, protocol and pathway development at Health Board level.

We actively support professional development and career progression, including access to non-medical prescribing training, advanced clinical practice development, leadership opportunities and involvement in audit, quality improvement and research activity.

Main duties of the job

You will utilise advanced clinical pharmacy knowledge to provide expert medicines optimisation for patients with cardiovascular disease, supporting safe, effective and patient-centred care across inpatient and outpatient settings. You will be encouraged to contribute to service development, quality improvement initiatives and review of clinical practice to support continual improvement in cardiovascular medicines use.

You will be supported to develop the scope of the role, working across the Central IHC alongside the Heart Failure team, cardiac rehabilitation services, and the Emergency Quarter and wards, reviewing patients with cardiovascular disease admitted to hospital. You will be supported to prescribe and monitor bespoke and complex treatment regimens, delivering safe and effective outcomes with a patient-centred approach, while using or working towards independent prescribing.

This role is ideal for a motivated and forward-thinking pharmacist with excellent communication, time management and interpersonal skills. You will be self-motivated and confident in your ability to liaise with and influence senior clinical staff, members of the multidisciplinary team and colleagues from a range of professional backgrounds across primary, community and secondary care within the Central IHC.
