An exciting opportunity has arisen for enthusiastic, ambitious and forward thinking individuals to join the Central Primary Care team supporting clusters across Conwy and Denbighshire. The position holder will have a particular focus on Conwy East.
Primary and Community Care is an exciting place to work right now. Welsh Government are investing in clusters and localities across Wales to serve the local population in a whole system approach.
The successful candidates will work alongside cluster leads and locality members to develop the roles and functions of localities in line with Welsh Government and Health Boards locality developments.
The co-ordinator role will be responsible for providing support and facilitation to clusters and localities, supporting the population needs through effective and robust planning and service delivery, whilst leading development at pace.
The post holders will also ensure primary care and wider locality services are developed and deployed effectively as a critical part of strong primary and community model of service delivery.

Main duties of the job
The main duties of the job will be:
Supporting the development of Primary Care and Community services through facilitating the development, documentation and implementation of Cluster plans.
Leading on the implementation of specific projects and areas of work.
Support Collaborative and Cluster meetings as required with logistics and governance.
Support the development of strong working relationships within the cluster network.
Collate and analyse the data required to make planning decisions and to monitor actions.
Developing and preparing written reports in a timely manner for Cluster meetings, Pan Cluster meetings and Area Team.
Communicate directly with Collaborative and Cluster members on phone, email, in person and presentations to small groups.
