EIP Practitioner - Band 5/6 Development Post

Job summary
The early intervention in psychosis service, East area team, are looking to recruit a motivated and dynamic professional practitioner, to work within the newly developed service. The post holder will undertake responsibility for the care co-ordination and case management of a defined caseload. The post holder will work in a multi-disciplinary team, working with people with a first episode psychosis, to support every aspect of their journey, enabling the person to have the best possible outcomes. The post holder will receive professional leadership support and line management, with opportunity for on-going peer support and advancement in training in EIP.
The post is a Band 5/6 training and development opportunity in the EIP service East, which will be covering 3 LA counties with the planned developments.
Main duties of the job
The post holder is required to work within an integrated EIP MDT focusing on adults and young people suffering with mental health problems that fall in line with EIP standards. The post holder will hold a caseload, being responsible for assessing patients' needs, planning and implementing the plan of care.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
