Job summary

Work with individuals who are in crisis and having difficulty coping, to prevent hospital admission where there is potential for providing acute intervention packages enabling the individual to remain at home.

You will hold a specialist case load as referred. You will be required to work flexibly, intensively and responsively to clients' needs by generating ideas and making recommendations.

Those individuals using the service have severe, enduring and highly complex mental health needs. The aim of intervention is to deliver a high standard of care in the home, offering an alternative to potential hospital admission. The post holder also facilitates early discharge from hospital to reduce inpatient stay by monitoring and managing the risk of looking after someone who is still acutely ill in their own home.

You are required to assess need, formulate, implement and evaluate care plans for their designated specialist case load and maintain associated records taking into account all aspects of risk management

Main duties of the job

The post holder will be a part of a multi-disciplinary team providing Home Treatment for people experiencing an acute mental health episode. One of the key roles is to gate-keep admissions to acute inpatient care and providing Home Treatment where appropriate. The team also seek to reduce inpatient stays by facilitating early discharge wherever appropriate.The individuals who receive a service from the Home Treatment Team have complex and acute needs. The aim of the team is to avoid hospital admission by delivering a quality service based on individual needs of the patient in the community. The role of the post holder will also to assess for suitability for early discharge from the ward to facilitate early discharge by conducting thorough assessment of risk prior to discharge.
