Metastatic Clinical Nurse Specialist (CNS)

Job summary

This post is fixed term for 6 months to meet the needs of the service.

The Metastatic Clinical Nurse Specialist (CNS) will demonstrate practice in both direct patient care and have the ability to support and develop services. Working alongside a band 7 Metastatic CNS and through the key skills of leadership, innovation and clinical expertise the CNS will facilitate and deliver patient centred care, which is efficient and effective.

The CNS is a core member within the multidisciplinary care team (MDT) and will work collaboratively with clinicians and service managers to promote, develop, review and redesign services which are responsive and relevant to patient's needs in line with national and local objectives. The CNS is an autonomous practitioner and has the freedom to act to influence patient care and service provision

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

  • To be a Key Worker for their caseload ensuring everyone receives a high standard of personalised care, working to coordinate care and provide easy access to services and ongoing physical and emotional support for patients their carers and families.
  • To be the patient advocate within the site-specific Multi-disciplinary Team (MDT).
  • Demonstrates leadership through clinical expertise delivering high standards of personalised care by acting as a key worker, identifying needs through holistic needs assessment and co-creating a care and support plan with people living with cancer.
  • Deliver Nurse Led Clinics utilising Holistic Needs assessment.
  • To establish working relationships and communication links between primary secondary tertiary and independent care providers to improve care for patients and provide specialist clinical expertise, advice and support when required.
  • Ensure that professionals adhere to the implementation and monitoring of relevant clinical guidelines, policies and procedures within their scope of practice and speciality.

Demonstrate critical thinking and diagnostic reasoning skills to differentiate between normal, variations of normal and abnormal findings taking appropriate action and enabling fast track assessment if required or expedite further action.
