Health Care Support Worker

Job summary

The post holder will help facilitate the smooth running of the day unit, which provides day services to individuals with dementia.

The post holder will work occasionally without direct supervision in the community, however, will have telephone access to an identified qualified nurse for advice and guidance if undertaking outreach work.

The post holder will promote and hold a positive attitude to dementia care and ensure that individual needs for privacy, respect, dignity and confidentiality is maintained at all times.

The post holder carries out assigned tasks involving direct care to those who have memory difficulties as directed by the Qualified Nurse.

To undertake interventions with people with memory related problems sometimes working independently within the day care setting and occasionally within the persons own home if required.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

  • Maintaining independence through prompting/assisting where necessary, with everyday activities such as shopping, cooking etc
  • Address issues of social isolation through taking part in activities occurring in the community e.g. coffee mornings, exhibitions etc.
  • Assist with personal care in cases were home care services are unable, due to there being a particularly good and trusting relationship with the client and they are willing for help.
  • Facilitating access to Social Services Day Care as some clients may initially be wary of attending day setting but respond well to a gradual approach.
  • Take to healthcare appointments in the local area.
  • Support residential homes. This would be short term input, e.g. when a client is living in a residential home but is waiting to move to an EMI residential home.
  • Assist a qualified practitioner to run therapeutic groups.
  • Increased support to a client during periods of stress (e.g. bereavement) or when a client is more confused than usual e.g. because of an infection.
  • Report back any problems or significant change in the individuals mental health presentation.
  • Write notes after every visit detailing what you did and how the client seemed to you.
