We are seeking a highly motivated, dynamic nurse to join our Home First Discharge Team in the WEST. If you are an enthusiastic, experienced registered nurse with a specialist interest in discharge processes, then we have the role for you. This is a new, exciting and challenging role for an experienced individual who is dynamic with the ability to think outside the box.

Main duties of the job

The successful applicant will join our Home First Bureau team to provide specialist knowledge and leadership to assist CRT/Hospital Staff in promoting effective discharge planning and implementing the Health Boards Strategy for Discharge to Recover & Assess (D2RA) across Gwynedd & Mon. The post holder should have a knowledge and understanding of The Discharge Process, Funded Nursing Care, and Continuing Healthcare & Social Well Bring (Wales) Act 2014. You should also have excellent communication, assessment and documentation skills and be committed to working within a multidisciplinary environment.

The post holder will work with colleagues across the Health Board; Local Authority & Voluntary sector to assist in developing and delivering models of care that enable people to leave hospital when they are ready, in order to provide care closer to home. In collaboration with others, will be instrumental in identifying themes and barriers to the discharge process and lead work streams to mitigate these. Will have excellent presentation skills and will have opportunities to deliver programmes of education and training.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

The role will predominately be based within the acute sector i Ysbyty Gwynedd but there will be an expectation to rotate to the community as needed.
