Are you passionate about proactive care and well-being? We're looking for motivated and compassionate individuals to join our dynamicFalls Prevention Serviceas aBand 4 Falls Prevention Practitioners on a permanent basis.
This is achallenging and multifaceted rolewhere no two days are the same. You'll be part of a dedicated team working with individuals who have fallen or are at risk of falling, with a strong focus onearly intervention and health promotion.
Our goal is to identify and reduce as many modifiable risk factors as possible to prevent further falls, reduce injury, and avoid unnecessary hospital admissions.
Main duties of the job
Under the direction of the service lead you will provide integrated person-centred interventions for a varied caseload for service users within Conwy and Denbighshire. The role will involve undertaking home visits to complete multifactorial falls assessments, identifying risk factors and prescribing appropriate assistive equipment, minor adaptions and making onward referrals. We also provide evidenced based (FaME/PSI) strength and balance programmes across Central BCUHB, and you will be an integral part of developing and delivering these sessions. You will also represent the service at a variety of public events and deliver talks to both the general public and staff to promote healthy ageing and falls prevention strategies.
We are looking for someone with previous health or social care experience who has excellent communication skills and the confidence to speak in public. Being proactive and empathetic are also essential attributes, as well as being IT literate.
The ability to speak Welsh is desirable for this post. Welsh and/or English speakers are equally welcome to apply.
