Acute Liaison Nurse

Job summary

Are you a motivated, registered Learning Disability nurse or a registered Adult/general nurse with knowledge and skills around the health needs of people with Learning Disabilities and would like to join a team whose priority is to improve access to health services for adults with Learning Disabilities. This role, primarily, will be to work alongside acute/general hospitals, the Health Liaison Team, service users, families and other professionals to ensure equality of care for people with Learning Disabilities, and to improve the quality of care within hospital services.

The post holder would be expected to be a member of the Health Liaison Team which works across BCUHB, the nurse would be based in Ysbyty Gwynedd hospital but would need to provide cover to Ysbyty Glan Clwyd as required.

Main duties of the job

To support people with Learning Disabilities accessing mainstream Acute hospital services and work alongside acute services to ensure reasonable adjustments to their care.

The Health Liaison Team work across North Wales , working with stakeholders to improve access to mainstream services for individuals with Learning Disabilities.

This role, primarily, will be to work alongside acute/general hospitals, the Health Liaison Team, service users, families and other professionals to ensure equality of care for people with Learning Disabilities, and to improve thequality of care within hospital services.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
