Job summary
- The post holder is responsible for the implementation and monitoring of Continuing NHS Health Care funding (CHC) in Care /Residential Homes and domiciliary settings in accordance with the national Framework for CHC and NHS Funded Registered Nursing Care (FNC).
- The role will involve extensive joint working with the Conwy/Denbighshire Local Authorities ensuring patients funded by BCUHB receive a high quality and safe service.
- The post holder will co-ordinate, scrutinise CHC applications, attend and participate in Multi Disciplinary Team (MDT) meetings within the Central Division where required.
- The post holder will manage the local resolution process for the OPMH Team - meet and liaise with families to discuss panel outcomes when individuals challenge eligibility decisions.
- The post holder will be responsible for the local resolution process within the CHC Team for Inter Agency Disputes - meeting and liaising with the appropriate Team Manager from the Local Authority where there is a dispute regarding eligibility for CHC funding.
- The post holder will work in collaboration with the statutory agencies where safeguarding issues have been identified and will attend POVA,/Strategy meetings.
- The role will involve working collaboratively with Community Psychiatric Teams; Discharge liaison Teams to ensure patients are discharged/transferred in a safe and timely manner
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
Work across professional and organisational boundaries in particular with Independent Care Homes, Social Services staff and other Health Boards and Primary Care Trusts when patients are moving in/ out of the area.
Develop and maintain sound relationships within the Statutory/Voluntary/Independent sectors including, health care professionals, GPs and in particular Local Authority areas across BCUHB.
Promote excellent customer care throughout the Continuing Health Care process ensuring local resolution of any concerns where possible. This will include providing and receiving highly complex, sensitive and contentious information requiring agreement or co-operation where there are barriers to understanding. This relates to external organisations and the public.
Negotiate with clients/families and members of the public delivering complex information in highly sensitive and emotional situations such as the outcome of Continuing Health Care applications/reviews which may have a financial implication in particular to self funding clients which is often met with hostile and a highly emotive atmosphere that requires skilled negotiation.
Maintain accurate case records and ensure that panel decisions are communicated clearly and sensitively to clients/families within an agreed timescale as per The National Framework for Implementation in Wales
