Job summary
An exciting opportunity has arisen for a motivated individual to join the Community Falls Team working 12 hours per week.
The specialty of the falls service is to offer high quality, evidenced based practice to identify those at risk of falls and opportunities to employ early interventions. A knowledge and understanding of falls prevention is desirable.
Main duties of the job
Applicants will need to be flexible in work bases including Wrexham and Flintshire and the ability to work well in a team and also alone.
The work will also include offering guidance and advice to Care Homes in Wrexham and Flintshire, in line with pre-determined training programmes as identified by the Community Falls Lead.
The ability to communicate with MDT partners, including the ability to deliver a pre-prepared training session to a small group of Care Home staff.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

- Level 4 vocational qualification in a relevant health care associated subject / Certificate in Health Care Support Worker Education
- GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications
- Evidence of continuing personal development relevant to the role in health care
- Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines and the all Wales Band 4 Assistant Practitioner Governance Framework.
