Job summary

An exciting opportunity has arisen for a Band 3 Dementia Support Worker to work on AMU Assessment which is part of the Emergency Care Directorate in Wrexham

The new Dementia Support worker post holder will work as part of the multi-disciplinary team in order to plan, organise and facilitate a comprehensive range of individualised, group and one to one activities for patients admitted on to the unit.

Main duties of the job

The post holder will be predominately based on AMU Assessment . The successful candidate will promote and encourage appropriate, individualised level of patient participation and facilitate patient activities that promote social interaction, enhance self-esteem and help to maintain or enhance functional ability.

Applicants for this vacancy need to have evidence ofpatient participation and the ability to care with warmth and understanding. The Band 3 Dementia Support Worker is permanent and the post will be predominantly working Monday to Friday but a flexible approach will be required. The role will be supporting nursing staff to ensure that the patients receive the best possible outcome and experience during their hospital stay.

The successful post holder will be required to support the Ward Senior Sister / Charge Nurse & Ward nursing team through ensuring high standards of care as outlined in the Welsh Government Fundamentals of Care.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
