Job summary
The post holder will be required to manage chronic and acute presenting patients registered with the practice. These patients may be experiencing new onset undifferentiated, undiagnosed symptoms or acute exacerbations of previously diagnosed long-term health conditions. The post holder will be an autonomous practitioner, expected to work independently in a holistic manner, utilising skill and experience to provide comprehensive care from assessment through to treatment and, where appropriate, referral to other professionals.
The post holder will be expected to see patients in a variety of settings including, but not limited to, Primary Care clinics, patients own homes, residential or nursing homes. The post will be embedded within primary care and the post holder is expected to work at an advanced level developing new and innovative ways to meet the challenges of the position and the needs of the practice population.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The post holder will plan, lead and be responsible for the delivery and development of practice in the specialist area. They will fulfil the 4 pillars of advanced clinical practice: Clinical leadership Expert clinical practice Research and Audit activity Education
To not only treat ill health but to promote health, wellbeing and self-care thus empowering patients to achieve optimum future health outcomes.
To demonstrate advanced knowledge /skills and clinical reasoning to deal with complex cases within Primary Care.
To demonstrate advanced critical thinking and analytical skills to support clinical judgment and decision-making process.
