Working in an integrated way within primary care you will support the development of nutritional knowledge and behaviour change skills of health care support workers and dietetic assistants to deliver brief intervention for those at risk of type 2 diabetes. The role is fully supported by the national programme lead and team with programme protocols, resources, training courses for support workers and evaluation tools.
Whilst experience of working within Diabetes will be advantageous, it is not essential for this role.
Main duties of the job
Clinically lead the development, implementation, delivery and evaluation of best practice in relation to diabetes prevention for West IHC working as an integral member of and in partnership with primary care and the national programme team.
Support others in the delivery of highly specialist seamless care to individuals and groups who are at high risk of developing diabetes; supporting management of complex cases and promoting empowerment of individuals to self manage their condition.
Team leads delegated staff ensuring consistent evidence based standards of care and effective multi-disciplinary working across BCUHB.
Work with the other team leads within the area to ensure that there is consistent leadership of staff - particularly in times of leave or sickness. Forge and develop relationships within the other areas of the health board.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
