Job summary
An exciting opportunity has arisen within the South Gwynedd Older Persons Mental Health Team based in the Dolgellau and District Hospital .
We are looking to recruit an enthusiastic and motivated Registered Mental Health Nurse to join our established team caring for Older People based in the Meirionnydd area. Our friendly and supportive multi disciplinary team provide a community-based service , day service and outreach service to those with memory impairment and dementia diagnosis.
We are looking for someone who has a genuine interest in working in the Older persons service and be compassionate in meeting the needs of our service users and their carers .In return we can offer an in depth induction , supervision and full support as a valued team member and guide you in developing your career progression through regular performance appraisal.

Main duties of the job
The post holder is required to work within an integrated MDT focusing on older adults suffering organic mental health problems (dementia). The post holder will hold a caseload, being responsible for assessing patients' needs, planning and implementing the plan of care.
The post holder will provide supervision, guidance and support to junior members of staff.
To provide comprehensive psychiatric nursing assessment by being involved in the discussion and allocation of referrals to the CMHT. As well as provide nursing assessments for the LHB and Social services. Considering the criteria for funding i.e. CHCF, FNC and social services
To be responsible for the care coordination of a designated caseload of clients with organic mental health problems.
