Senior Community Nurse for Children with Disabilities

Job summary
We are pleased to be recruiting a B6 senior Community Nurse to a 37.5 hour permanent post, working in the community with children and young people with significant global developmental delay, social communication difficulties or learning disabilities and their families, as part of the Flintshire Children's Pre-school Development Team (PDT) and Community Learning Disability Health Team. Please note the post holder will be required to work flexibly across both the preschool development team and the school age learning disability team. The post holder will be primarily based in Flintshire, working with the Flintshire families.
We are looking for an enthusiastic and motivated individual who wants to work with pre-school children with developmental delay, and children with learning disabilities and their families, and who has prior experience of working with children.
A range of community work will be undertaken, including assessment for Autism for under 5's, as part of a multi professional team, specialist assessment to formulate interventions, strategies and care plans. These interventions can be delivered through group work; individual 1:1 work; working as part of the MDT as well as alone; and the successful applicant will need to be familiar with the Continuing Care Health funding process.
Service development is an active part of the role.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
- To take responsibility for allocated referrals from source to discharge in line with agreed protocols and documentation including assisting Clinical Psychologist, Psychiatrist or Advanced nurse practitioner in diagnosis of ADHD . This may involve assistance in interpreting complex cognitive assessment information or conducting assessments indicating of levels of functioning or undertaking developmental histories for younger children with developmental delay.
- Required to use the highest level of communication skills when dealing with clients who may be hostile or antagonistic. Required to adapt communication style to reflect the client's needs and understanding or be able to engage with younger children with severely limited social communication skills
- To conduct initial assessments with carers to agree suitability of the service to meet the child or young person's needs and agree outcome focused goals to work towards.
- To undertake an initial assessment of physical, psychological and social development and well being in order to accurately identify the child's needs and associated health / developmental needs.
