Job summary

Betsi Cadwaladr University Health Board in North Wales is a dynamic and patient focused organisation. The Health Board are responsible for the direct management of a number of GP Practices and are developing these practices to be gold standard exemplars, creating services that are better able to meet the needs of the local population, easier to access and navigate, and are a rewarding and satisfying place to work.

The Health Board is looking for a Nurse Practitioner who believes in a vision for high quality clinical care and compassion within Primary Care, and who wants to engage in delivering this in Pen Y Maes Health Centre, Wrexham. The Practice has an extended team consisting of a Clinical Lead GP, locums GPs, Advanced Nurse Practitioners, Practice Nurses, Health Care Assistants, Practice Pharmacists, and Pharmacy Technicians.

We are looking for a Nurse Practitioner with experience in Primary Care and dealing with Chronic Disease Management. Applicants will be a registered first level Nurse with evidence of study at MSc level.

The post is a fixed term contract for a duration of 12 months to cover Maternity Leave.

Main duties of the job

The post-holder will be expected to undertake all the normal duties and responsibilities associated with a Nurse Practitioner working within Primary Care, including (but not limited to):

  • Development and delivery of high quality patient care within primary care practices
  • Develop opportunities for practice, which extends the scope of traditional nursing
  • Training and support of nurses, patients and the practice MDT to deliver high quality, evidence based service provision
  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • Demonstrating through day-to-day action a contemporary approach to Primary Care which is patient-centred, prudent and team-enabling.

This role will require flexibility to meet the demands of the practice and an individual work plan will be agreed in advance with the successful applicant.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
