Job summary
An exciting opportunity has arisen for a suitably experienced and motivated individual, to work as a Band 4 Falls Practitioner within the Falls Prevention Service in the BCUHB Central IHC covering Denbighshire. This post is 37.5 hours per week on a permanent basis.
The post holder will plan, provide and observe outcomes for individual and group interventions which support the ongoing assessment and treatment plans of the service users in various community settings.
Main duties of the job
The team works with people who have fallen or are at risk of falling. The team's aims are to identify and minimise falls risk to promote well-being, prevent injury and unnecessary hospital admissions.
The role will involve undertaking home visits to complete multifactorial falls assessments and falls interventions including supporting the delivery of strength and balance exercise programmes and the provision of equipment and adaptations to reduce falls risks. The post holders will work autonomously under the direction of the service lead to provide an integrated service for service users within Conwy and Denbighshire. Being motivated, IT literate and having good communication skills are essential; Full support and training will be provided.
The ability to speak Welsh is desirable for this post. Welsh and/or English speakers are equally welcome to apply.
