Job summary

We are seeking an experienced and proactive Band 6 Occupational Therapist to join our dynamic in patient team in Glan Clywd Hospital,

This role will involve working as part of an integrated, multi-disciplinary team to provide assessment and treatment to patients who present to the acute setting via an older persons' pathway.

Main duties of the job

You will be able to demonstrate specialist knowledge and skills in treating older people as well as having the ability to work autonomously and as part of team.

You will work to a discharge to recover and assess model in order to reduce the risks of hospital-related deconditioning and maximise independence with this patient cohort.

You will have excellent organisational and time management skills and be able to work flexibly to meet the demands of this busy service.

Your responsibilities will include supporting junior team members with both informal and formal supervision, providing opportunity to develop your skills in clinical leadership whilst being supported by the senior Occupational Therapy Team. You will also have opportunity to be involved with exciting service developments as we further develop discharge to recover and assess pathways for older people from the point of admission.

If you are passionate about the care of the older person within the acute hospital setting along with wanting to support the development of an excellent Occupational Therapy service within this setting then this is the role for you.
