Are you an Independent Prescribing Nurse Practitioner looking for a role that will give you variety, challenge and a supportive team? We have two posts to join us based in the community hospital in Tywyn in Wales and working with both minor injuries and minor illnesses supporting the south Meirionnydd cluster. You will work as part of a professional team, supported by the urgent primary care clinical team and the wider local teams and play a key role in providing patients in the area with accessible and timely local prescribing.

The new Minor Treatment Centre situated in Tywyn is a transformative new service that provides an alternative pathway for patients with acute onset minor conditions or minor injuries, reducing pressure on Primary Care and Emergency Departments. As a part of the team there you would have the opportunity to make a real difference to our local patients.

Main duties of the job

As an Independent Prescribing Nurse Practitioner you will be working as a member of the Minor Treatment Centre team to provide care to patients referred from Primary Care, ED, Out of Hours and Community Pharmacies with acute onset, single issue, urgent care needs.

You will assess patients' clinical needs, providing safe and effective care through history-taking, physical assessment, diagnosis, and liaising where appropriate with other healthcare professionals. Central to the role will be the use of critical thinking and advanced decision making skills in order to accurately assess and manage patients with a wide range of clinical conditions/illnesses, this may be over the phone (or via other formssuch as video consultation) or face to face in the centre.

The candidate must hold an Independent Prescribing qualification for this post.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
