Job summary

The Primary Care and Community Directorate is seeking to appoint an enthusiastic and forward thinking Senior Cluster Locality Manager to work alongside the Area Management Team and partners in the West (Anglesey and Gwynedd)

The post holder will support and facilitate the development and implementation of the primary and community services strategy across the Area including the development of the locality cluster model of service delivery. The post holder will work with Cluster Leads and Cluster members to develop the roles and functions of Clusters in line with aspirations set within the Health Board's Primary Care Strategy and to embed cluster working

The post holder will be responsible for providing support and leadership to the Clusters and the Area Management Team to ensure that primary care services are developed and deployed effectively as a critical part of a strong primary and community model of service delivery.

As a member of the Primary Care Area team the post holder will lead on the development of new models of care in primary care and community services including the development and planning of the CRTs in partnership with public sector and third sector organisations.

This is is fixed term/secondment for 11 months due to funding

Main duties of the job

  • Support and lead the development and implementation of Cluster and/or Area based services to improve community health and well being in line with local, regional and national strategies.
  • Ensure that a range of programmes and projects relating to Cluster developments are managed, monitored and reported
  • Negotiate, influence and manage one to one and group meetings with a wide range of individuals across all sectors to effectively engage and support the delivery of Cluster based developments
  • Communicate and advocate for priority areas for development/improvement as defined within Cluster Plans to partners, including primary and secondary care, social services and the voluntary sector.
  • Develop effective working relationships with primary care practitioners local and National Public Health Wales and partners to promote and develop a collective approach to locality service planning and delivery
  • Ensure effective performance management in all areas of personal responsibility giving assurance to the Assistant Director on the achievement of programme objectives.
  • Manage complex information to produce concise reports, highlighting current performance, critically appraising delivery and proposing changes to improve performance.
