Job summary
An exciting opportunity has arisen for a Band 4 Stoma Care Assistant Practitioner to join our Stoma Care Department based at the Royal Gwent Hospital.
You will assist in co-ordination of stoma patient care within the stoma care department, manage basic complications of stoma care under the guidance of a Registered Nurse to facilitate patient's return to normal life.
You will provide holistic needs assessment and maintain the stoma care database.
Main duties of the job
o Support the Stoma CNS ensuring effective collaboration between departments across stoma pathway across multiple divisionso Support MDT to provide high quality, evidence-based person-centred care, e.g., supporting patients through results/ diagnosis and providesupport and sign posting to other agencies as required.o Contribute to developing and implementing a continued strategy for comprehensive stoma care.o Refer complex decisions to Stoma CNS for assessment and reviewo Sign post/ expediate or instigate referral to appropriate member of MDT i.e., benefits support/ dietician/ Stoma CNS/ Clinician.o A key aspect of the role involves daily and direct (e.g., face to face and telephone) tactful communication with patients, relatives, carers, andother health and social care professionals. The information and nature of the communication required is complex and sensitive due to the natureof disease and surgery.
o Communication in this context requires a high degree of empathy, understanding, diplomacy, honesty and integrity. o Undertake patient assessment and complete holistic needs assessment as required.o To contribute to the adjustment or modification of individual patient care plans and disseminate information to MDT.o To have a knowledge of various stoma appliances available to patients in both hospital and community setting and be able to give change to amore appropriate appliance when required.
