Job summary
Integrated Well-being Networks have been implemented across Gwent to improve well-being through community-based, place-focused approaches. They form a key part of the whole-system transformation set out in the Welsh Government's long-term plan for health and social care.
This role will lead the development of Caerphilly's place-based well-being approach--strengthening local networks, engaging partners, and working with community assets to promote well-being for all.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The Service Development Lead will play a central role in progressing place-based care across the county borough, including community engagement, capacity building, asset mapping, and coordinating well-being resources to support individual and community behaviour change.
The post holder will lead the development of approaches that better connect well-being resources across primary care, community and voluntary services, and will strengthen alignment between the Regional Partnership Board, Public Services Board, and Caerphilly's Integrated Services Partnership Board well-being priorities.
We are looking for someone with energy, commitment, and the ability to inspire partners and communities to work together and create positive, long-term change.
This is a fixed-term until 31 March 2027 due to funding. Secondments welcome.
