Job summary

The post holder will provide clinical leadership for community mental health services within Older Adult within a defined geographical area.

Responsible for ensuring delivery of high-quality, person-centred, safe, efficient and cost-effective services across community services.

Will deputise for the Operational Manager (Band 7) when they are unavailable, supporting clinical governance and professional leadership.

Main duties of the job

To carry the continuing responsibility for the assessment, planning, implementation and evaluation of care needs andquality assurance for the patients within their area of responsibility.o To line manage the nursing team (Registered & HCSW) ensuring the professional and clinical development of teammembers.o To work within the codes of practice and professional guidelines, accountable for own professional actions.o Provide specialist clinical leadership across community mental health pathways for older adults, including functionalmental illness and dementia.o Deliver expert assessment, formulation, diagnosis and care planning for complex older adult presentations, workingautonomously and holding a personal clinical caseloado Provide clinical supervision and case consultation for multidisciplinary colleagues across secondary care CMHTs andprimary care mental health interfaceso Lead on differential diagnosis of cognitive disorders and functional disorders, guiding evidence-based interventions andrisk management.
