Job summary
An exciting opportunity has arisen for an enthusiastic, motivated and forward thinking Operational manager. To provide operational management to the non-clinical functions of the Health Inclusion Service. The postholder will utilise local data and evidence to meet the needs of the most vulnerable groups in the most deprived areas across Gwent, focusing on the development of new and, the strengthening of existing pathways for the most vulnerable populations of Gwent to promote collaboration and integration between Tier 2 and 3 of the Health Inclusion Model.
The post-holder will work collaboratively with Urgent Primary Care, NCN Clusters and both internal and external partners across the five locality areas to develop outreach and drop in provision, ensuring that expertise and capacity is maximised through a coordinated and coherent model. The post-holder will drive forward the principles of place-based care via the management of Link Workers to ensure that there is a health inclusion presence within each cluster.

Communication and Relationship Management
- Establish and maintain strong working relationships with a wide range of partners across the Primary Care & Community (PC&C) Division
- Share complex and sensitive information with key partners and lead discussions to co-design or strengthen pathways
- Provide written and verbal reports on service delivery, performance, and operational issues.
Analytical & Judgemental Responsibilities
- Apply creative and innovative thinking to develop new concepts and solutions that address service challenges.
- Use strong analytical skills to gather, interpret, and collate data to assess whether service objectives are being achieved
- Identify operational risks and issues, recommend solutions, and escalate concerns
Planning & Organisational Responsibilities
- Support the Directorate Manager and Senior Nurse in identifying opportunities for ongoing service development aligned with strategic objectives.
- Manage, motivate, and develop administrative staff
Financial & Physical Resource Management
- Contribute to service budgeting, workforce planning, and operational planning processes.
- Monitor budgets and cost implications, identifying potential overspend, pressures, and opportunities for savings.

This role is responsible for developing and embedding a multidisciplinary team (MDT) process across all service areas to enable the effective receipt, discussion, and escalation of complex, contentious, and sensitive information. The primary aim is to ensure the needs of highly complex individuals are identified early and that appropriate, responsive pathways are established and maintained. The post-holder represents the Health Inclusion Service under the direction of the Service Lead and plays a key role in fostering strong working relationships across the Primary Care and Community (PC&C) Division, including GPs, partner health providers, local authorities, and external agencies.
A central responsibility is sharing sensitive and complex information with key partners and facilitating collaborative discussions that develop new pathways or strengthen existing ones. The role works closely with the Health Inclusion Leadership Team and Clinical Service Leads to oversee performance reporting, ensuring accurate monitoring of service demand, capacity, and key performance indicators. This includes analysing complex service issues, supporting the development of solutions, and presenting service improvements.
The post-holder provides written and verbal reports to a range of internal and external audiences, using negotiation, diplomacy, and tact to navigate challenging discussions, particularly where partner resistance or differing viewpoints may arise. They are required to support, motivate, and engage staff throughout pathway development, service change, and outreach activity, maintaining continuous communication with teams and partners to drive ongoing service development.
The role also involves managing difficult conversations, including those relating to performance concerns and changes to systems or processes. Clinical risks must be escalated via the QPS process to senior staff, ensuring robust clinical governance. Effective communication with staff and senior managers is essential throughout, ensuring transparency, clarity, and alignment across all areas of delivery.
