Job summary
The Aneurin Bevan University Health Board is looking to recruit experienced Nurse Practitioners to work their UPC 24-7 service.
Using effective telephone consultation and triage skills, successful candidates will assess and advise patients/callers on appropriate pathways to deliver safe and evidence based care. As an Independent prescriber successful applicants will become part of the existing multiple disciplinary team, assessing and responding to appropriate patients who require urgent care advice and further treatment.
Call outcomes will range from immediate referral to the emergency services by phone through to self care advice by phone. UPC 24-7 NPs are also expected to identify cases that can be rapidly triaged to attend their current UPC centre, focusing on appropriate appointment timings and treatment guidance. Such patients are then assessed and treated by NPs/ANPs (with patient age ranges from age of 12 years and up) from one of the three UPC 24-7 centres.
The core hours will be working the Integrated Front Door at Ystrad Mynach Hospital. This is a nurse-led service working alongside the Minor Injury team to see and treat non-injured patients who attend. There will also be an expectation to develop as a mobile response nurse for appropriate home visits. Full role specific training and support will be given during the induction period.
Main duties of the job
- Telephone consultation and triage assessing all clinical conditions across all age ranges
- Autonomous decision making in relation to these call outcomes (self care) and referrals internally within UPC or to other services - inside and external to ABUHB
- Face-to-Face consultations in Urgent Primary Care Centres and autonomous decision making in relation to appropriate care and treatments. This post predominately focuses on the "Integrated Front Door" service in the Ysbyty Ystrad Fawr (Ystrad Mynach)
- Other clinical and leadership duties along with service developments as per job description.
