Rapid Frailty Response Nurse

Job summary

The Rapid Response Team plays a vital role in admission avoidance, delivering a 'hospital at home' model of care for individuals experiencing acute illness. The team also supports early discharge from The Grange University Hospital, working closely with the Front Door Frailty Team to ensure seamless transitions from hospital to home.

We are seeking a dedicated and compassionate Band 5 Registered Nurse to join our Rapid Frailty Response (RFR) Team. The ideal candidate will be committed to delivering high-quality nursing care within community settings, ensuring patients receive personalised support and treatment tailored to their individual needs

The team operates as part of a widermulti-disciplinary network, including General Practitioners, Consultants, District Nurses, Physiotherapists, Occupational Therapists, Community Psychiatric Nurses, Social Workers, Support and Well-being Workers, and the Voluntary Sector.

Key Responsibilities:

  • Deliver high-quality, person-centred nursing care in the community during periods of acute illness.
  • Facilitate early discharge from hospital by working closely with theGrange Front Door Teamand community services.
  • Provide timely nursing assessment, care planning, implementation, and evaluation.
  • Promote health and prevent illness through proactive interventions and education.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job

  • To be responsible for the assessment, planning, implementation and evaluation of care for community-based patients.
  • To work as a member of the wider primary care and community multi-disciplinary team.
  • Administration of IV therapies within the community and CRT clinics
  • Delivery of core nursing care; Subcutaneous and IM injections, wound management, bladder, catheter and bowel care.
  • Undertaking basic and enhanced observations (ECG's)
  • Supporting RFR Medics and Nurse Practitioners to complete comprehensive geriatric assessments
