Job summary

The post holder will support the work of the MSK pathway in the development of supported self-management approaches such as ESCAPE Pain and Good Boost across the Health Board.

The post holder will take responsibility for collaborating with Health Board clinical teams, Local Authority leisure and National Exercise on Referral Teams (NERS) to support the rollout of Good Boost as a self-management approach to people with MSK conditions.

Will be responsible for supporting people to access supported self-management options in the community.

The post holder will plan their own workload and work independently to defined policies and procedures and be responsible for managing own workload.

This post is fixed term/secondment for 24 months due t meet the needs of the service.

Main duties of the job

The successful candidate will support the roll out of supported self-management for MSK conditions across Local Authority and Health Board teams, reviewing activity and utilisation data and providing updates to MSK Service Lead.

Working with the MSK Therapy teams, local authority leisure and National Exercise on Referral (NERS) teams, the post holder will be initiating and establishing new systems of work to improve efficiency and effectiveness. Therefore they will need to develop clear, organised systems to report and respond to utilisation and outcome data.

The post holder will be supported as part of a close knit MSK Therapy team and utilise their communication skills to develop relationships between health and leisure services and support citizens to access this self management option in their local leisure facility.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
