Job summary
Aneurin Bevan University Health Board is seeking a dynamic and experienced Programme Manager to join our Transformation and Delivery Team. This is a pivotal role supporting the design and delivery of the Place-Based Care (PBC) Programme. PBC is central to both A Healthier Wales: Our Plan for Health and Social Care and The Primary Care Model for Wales, which advocates for a shift in focus from a traditional hospital-based medical model to one that is focused on prevention and early intervention. The PBC programme will bring together community members, organisations, services, and teams responsible for health and well-being of the population and communities.
Main duties of the job
As a key member of the team, the post holder will lead the design, planning, and delivery of complex transformational system change across community, health and social care. Working in close collaboration with Divisional teams, Public Health, Primary Care, Community Services, and wider system partners, you will drive forward innovative and sustainable system change aligned to the Health Board's Integrated Medium-Term Plan (IMTP) and 10yr Better Health, Better Care Better Lives, Gwent 2035 strategy as well as the priorities set by the Gwent Regional Partnership Board.
You will be responsible for identifying and shaping opportunities for transformational change, supporting Divisions to implement new ways of working that are person-centred, informed by population need, are outcomes-focused, and system-integrated. This includes providing hands-on programme and project management support, facilitating stakeholder engagement and involvement to harness a collaborative approach that aligns activity towards a shared vision and purpose with people and communities at the centre, and ensure the delivery of measurable change using robust monitoring and evaluation tools.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

- Lead and manage a specific portfolio of programmes/workstreams/projects, such as transformative service models, organisational development, productivity and efficiency opportunities which will require cross health system design and delivery.
- Ensure all transformation plans are holistic and that they are centred on the evidence-based needs of patients and population.
- Responsible for ensuring programme plans bring together all elements of design and delivery including overall service design, workforce, capacity and demand, system and process design, technology, capital and finance.
- To represent the Place Based Care programme in the relevant Divisions, or corporate department relating to ensure that areas plans are transformative, robust and meet the population need.
- Lead on specific developments which improve the project and programme skills and capability across the Health Board, including the development of project resources in an easily understood format, encouraging and supporting their proactive use by all appropriate staff within the organisation.
- Provide direct support to key lead staff at a division level, for example, General Managers, Directorate Nurses, Directorate Managers and Divisional Directors, in the planning and redesign work associated with the post lead responsibilities. This will also include working closely with Primary Care Leads in the implementation and monitoring of local plans and clinical pathways.
- Assist healthcare professionals to use project management methodologies and techniques to gain the maximum benefit from operational processes.
- Lead on current and future developments in transformation programme development, which support the delivery of services in line with the Gwent 35 strategy.
- To demonstrate a high standard of professionalism, efficiency and effectiveness in the delivery of project and programme support, ensuring that activity is fully aligned to supporting strategy.
- To spread and facilitate a culture of continuous improvement and professional excellence across the organisation.

- A high degree of flexibility is required as the type of transformation programmes could vary significantly from those that have a strong capital ethos through to those that are more efficiency focused through to service design and anything in between.
