Job summary
The role is based out of County Hospital or YAB and the post holder will travel across ABUHB in the community to assess and treat patients in their own homes. The post holder will work alongside a HCSW and is overseen by a Team Manager.
Main duties of the job
Assess and deliver care to patients in the community under the direction of the team leader.
Maintain accurate record keeping and standards for documentation including IT systems
Ability to work across the 24hr community nursing service.
Have competence in a range of clinical skills needed for community nursing.
The OOH Community team are based from County Hospital and YAB.
The night shift team cover all of ABUHB incorporating all the boroughs within the health board. Day shifts will be linked to a specific team in a single borough.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
