Job summary

Opportunity to join the South East District Nursing Team on a permanent contract as a band 3 HCSW a vital and valued role within the team

To support registered nurses in delivering best practice evidenced based care to patients within the Community Nursing Team. The Health Care Support Worker will assist the registered nurse/associate practitioner in the provision high quality, consistent and cost effective health and social care. The Health Care Support Worker will be responsible for assisting with the delivery of patient centred care as delegated by the registered nurse/associate practitioner within their scope of practice.

Main duties of the job

Job responsibilities

  • Ongoing holistic assessment for patients with long term conditions as required as part of patients ongoing package of care for those that are housebound or find it difficult to access regular healthcare.
  • To implement care as planned and directed by the Registered Nurse, reporting any changes in the patients condition in a timely and appropriate manner.
  • Carry out direct care duties under the delegation of a registered nurse e.g. phlebotomy, vital observations using NEWS, sample testing, wound care for grade 1 and 2 classified wounds.
  • To establish and maintain effective working relationships with internal and external agencies interacting with patients and colleagues in a professional manner.
  • To maintain written records of care in accordance with ABUHB policy.
  • To maintain IT systems, including inputting and extracting information.
  • To promote health, prevent illness and encourage self care as appropriate/applicable taking account of the patients choice and preference.
  • To recognise and escalate any concerns or observations regarding safeguarding to ensure a prompt and timely response, having regard for the safety and welfare of the patient within their home environment.
