Job summary
We are seeking a passionate and motivated Band 6 Integrated generic chronic conditions nurse to join our community nursing team based in the Chepstow District Nursing Service providing care in Chepstow and the surrounding areas. The District Nursing Service operates as part of an integrated health and social care service. We work closely with GP'S and other members of the multidisciplinary team to provide an excellent standard of person centred nursing care to adult patients in their own homes, supporting them to self manage where possible. We also provide a Frailty service as part of our District Nurse team. You will need to be a team player but confident at working alone in the community setting and working as part of a team. We expect excellent communication skills. This is an integrated role providing Chronic condition patient care and District Nursing.
Main duties of the job
This role involves visiting people in their own homes to support and review their chronic condition disease. We are an integrated service so the role will include working as part of the district nurse team.
Skills and knowledge with chronic condition management will be beneficial.
We will support you to transfer your existing clinical knowledge and develop skills to become a successful Band 6 Integrated generic chronic conditions nurse.
Applicants will be required to participate in working other shifts as part of the role including late shifts, bank holidays and weekends.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply
