Job summary
To assist with the assessment, planning, delivery & evaluation of patient care. To provide prescribed nursing care without direct supervision.
To contribute to the development/evaluation of nursing practice on the ward. To act in accordance with the NMC Professional Code of Conduct and other policies.
To provide nursing care across all areas of health care delivery within the Monmouth locality as required by the service.
Main duties of the job
An exciting opportunity has become available for a part time Band 5 Registered Nurses to join the Nursing team on Cas Gwent Unit, Chepstow Community Hospital. We are an innovative ward providing care to patients with rehabilitation needs and at times palliative needs. We are a well established friendly team looking for new members to help us to delivery excellent patient care.
We are focused on discharge planning and have many complex discharge requiring excellent communication and discharge planning skills. We pride ourselves on giving excellent patient care putting patients and their relatives first.
We are a 32 bedded unit with both male and female patients. We work closely with the integrated service teams and have excellent working relationships with the other MDT members.
We are a dynamic community ward and have taken part in various patient collaborative's and are always looking for new ways to improve on the care we provide. We aim to deliver high quality care with in the progressive environment of an integrated health and Social Care model.
We a friendly welcoming team looking to support our staff to develop in their roles and be the best they can be.
We care for our patients 7 days a week, 24 hours a day working 12 and 6 hour shifts.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
