Job summary
The Tissue Viability Clinical Nurse Specialist is expected to work in a variety of clinical settings including community and primary care.
Duties and Responsibilities of the post include; responsibility and accountability for the assessment, planning, delivery and evaluation of care programmes for the management of a wide range of patients with tissue viability issues. The issues may result from diverse, complex health problems including, diabetes, rheumatoid arthritis, neurological problems, circulatory deficits and immunosuppression.
The TVN Team are based at Llanfrechfa Grange Hospital but work in a variety of clinical and non-clinical settings.
Regular clinics are held at community hospitals, the team visit patients in their own homes in addition to nursing/residential home patients which includes teaching as well as assessing and treating. As such the post holder will be expected to travel as part of this role.
There are educational and development opportunities which apply to CPD and re-validation for the NMC.
Main duties of the job
The post holder is expected to manage and prioritise a clinical caseload of clients with complex wound care needs using evidence based, client centred principles to assess, plan, implement and evaluate interventions in a variety of settings, including wards, out patient clinics and community hospitals.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
