Job summary

The ABUHB Flow Centre is an innovative service that has been set up to ensure that patients in the ABUHB area receive the best care to meet their needs. The Flow Centre is one of the first points of contact for Health Care Professionals accessing secondary healthcare in the ABUHB area. It primarily receives referrals from Primary Care and the Welsh Ambulance Service Trust (WAST) along with other referring agencies. The post holder will be accountable for the continued development and delivery of the ABUHB Flow Centre Service. The post holder will develop pathways and protocols via collaborative working that the Flow Centre staff can use to ensure patients are directed to the appropriate care facility for their presenting condition. The post holder must be a Registered Nurse and is therefore bound by the Nursing and Midwifery Council (NMC) Code of Professional Conduct for Nurses, Midwifes and Health Visitors.
Main duties of the job

The post-holder will be expected to:
- Provide visible senior clinical leadership and provide a high degree of personal and professional autonomy.
- Make critical judgments to satisfy the expectations and demands of the role and risks.
- Work flexibly as part of the team to meet the needs of the service covering nights, weekends and bank holidays, as needed.

Plan the delivery of the Flow Centre Service to ensure effective and safe service delivery.

Work to minimise the number of inter site transfers, ensuring the patient receives the most appropriate care as quickly as possible.

To co-ordinate the effective utilisation of resources to ensure that there is a logical flow of patients throughout the system to have the right patient in the right place at the right time in a safe environment.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
