Job summary
An exciting opportunity has arisen within the Caerphilly Older Adult Community Mental Health Team to join the team as a Band 6 Community Mental Health Nurse working within the Caerphilly Care Home Liaison Service where you will be allocated a case load of Care Homes within the Caerphilly Area and you will be the main point of contact for mental health reviews of patients within the allocated homes.
The successful applicant will be employed with a permanent contract of 37.5hrs a week.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
