Torfaen Community Resource Team - Emergency Care @ Home

Full-time and Part-Time Hours Available

We are looking for an enthusiastic and driven individual to join our Emergency Care at Home Team (EC@H). As a community carer in the EC@H team you will work under the guidance of the EC@H management team, to achieve the service aims. You will be working collaboratively with other providers in the community to ensure the needs of the individual are met. You will also be a part of our amazing vision of bringing care closer to home.

Main duties of the job

The post holder will visit people in their own homes to provide care and interventions prescribed by registered health and social care professionals in accordance with their shared Support Plan. This work will be supervised by registered professionals but post holders are expected to work independently when visiting the person.

You will be working as part of the wider Community Resource Team to ensure a plan is developed and delivered that meets all the person's health and social needs.

The post holder will be responsible for delivering the plan in collaboration with the Community Resource Team, the person and their carers. The plan will be altered in response to the changing needs of the person and their carers.

The opportunity to gain extended skills such as phlebotomy are also supported within this role.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
