Job summary
An opportunity has arisen for an enthusiastic, self-motivated individual tojoin our team at Blaenau Gwent Rapid response.
The successful candidate should possess good leadership skills, be able to work well under pressure, possesseffective communication skills and willing to embrace change. Have experience in patient assessment to support community referrals.
They will carry the continuing responsibility for the assessment, planning, implementation and evaluation of care needs and quality assurance. The community nurse team leader will line manage a nursing team ensuring the professional and clinical development of team members. They will promote evidenced based practice and act as an accountable role model. They will ensure that together with their team members they act in accordance with the NMC Professional Code of Conduct and other relevant policies
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
- To provide day to day line and operational management for a team ofregistered and unregistered nurses
- To support team member development and the undertaking of staffappraisals ensuring all staff have personal reviews and developmentplans cognisant of service needs
- To support the development of a continuous service improvementethos, challenging traditional methods of core delivery, supportinginnovation and appropriate change within the nursing team
- Allocation of calls
- screening referrals from GP and other professionals, taking referrals and ensuring allocation to appropriate staff
- Reviewing and support annual leave of team
- managing sickness
- Nursing Team Manager
- To actively participate in risk management and clinical governancerelating to the Teams practice
- To contribute to policy development and changes in practice includingthose that may impact on other areas.
- To assist the team manager in the control of allocated budgets, toenable the best service provision within the resources available
- To provide expert clinical advice and support to team members
- To work as an autonomous practitioner and be professionally and legally accountable for all aspects of their professional and clinical work, within agreed national local and professional standards and guidelines
- To ensure the acceptance of referrals from other disciplines and co-ordinate intensive and complex packages of care with other multi disciplinary agencies
