Job summary

The post holder will ensure the provision of cost effective, efficient and appropriate high quality patient care in the community. They will carry the continuing responsibility for the assessment, planning, implementation and evaluation of care needs and quality assurance. The community nurse team manager will manage a nursing team ensuring the professional and clinical development of team members. They will promote evidenced based practice and act as an accountable role model. They will ensure that together with their team members they act in accordance with the NMC Professional Code of Conduct.
Main duties of the job
To manage a team of district nurses out of hours and provide safe and effective care to our service users in the community.
Managing day to day operational needs, staff sickness/absence, PADR's, e-rostering, incident reporting and RCA.
Ongoing service improvement and working collaboratively with the GP surgeries within the local NCN.
The OOH District Nursing Team consist of a Team Manager x 2 band 5 district nurses, band 4/3 HCSW.
The team work collaboratively with other MDT Professionals in addition to working closely with service users and their families.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
This post is secondment for 12 months due to - meet the needs of the service.
