Job summary
Applications are invited for a full-time Senior Clinical Fellow post at Aneurin Bevan University Hospital.
This is a fixed-term post for 12 months with the potential to extend to 18 months depending on service need.
Main duties of the job
This post involves working in the Cas-Gwent Unit at Chepstow Community Hospital providing active medical input to patients admitted for stroke rehabilitation, rehabilitation or palliative care needs. The Cas-Gwent Unit has 32 beds and also manages direct admissions from local General Practitioners. Palliative support is provided by St David's Hospice.
As part of the Clinical Futures model there will be a reconfiguration at the site with the introduction of five Community Frailty Unit beds and a Community Resource team. It is anticipated that there will be an interface with the CRT team and associated nursing staff. The ward cover comprises a Consultant, speciality doctor and a Senior Clinical Fellow (this post).
The successful candidate will become a valuable and senior member of the Frailty and Community COTE Directorate and will be responsible to the Consultant Physician for whom they are working. The expectation of the role is to provide senior support as part of a multi-disciplinary team. The post holder will be expected to participate in outpatient clinics related to the specialty and take part in multidisciplinary meetings within the department. The post holder will carry out independent ward rounds.
The post is part of a team of middle grade doctors within the Frailty and Community COTE Directorate based at Chepstow Community Hospital.

- Completing discharge summaries within 24 hours (one working day) of discharge of all patients
