Job summary
We are looking for a forward thinking individual to join our Commissioning Team for Mental Health and Learning Disabilities. The post holder will ensure the provision of seamless, integrated services or package of care for individuals who meet continuing healthcare criteria or are subject to 117 that require commissioned services that cannot be met by core services. They will have a key role in the implementation of the National Framework for Continuing Healthcare within the mental health and learning disabilities division and will provide a case management function to a caseload of patients / service users eligible for CHC.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
To attend and be part of an individual's MDT where there is a potential requirement for a commissioned service.
To support the team in applying the correct process for Section 117 Mental Health Act 1983, Continuing Healthcare Framework 2021 or the Additional Learning Needs (Wales) Act 2018.
To ensure that all of the governance is in place to ensure the correct placement is chosen.
To monitor placements to ensure that quality and safety are assured.
To support colleagues within the team and wider Division in ensuring robust evidence based support is available to them in applying for placement funding.
To adhere to policies and processes in relation to Commissioning Activity.
