Rehabilitation Assistant - Community Resource Team

Job summary

An exciting opportunity has arisen to join our well-established Community Resource Team, based in Cardiff. You will be working as part of a dynamic Therapy team delivering rehabilitation programmes to patients within a Care Home reablement unit setting and the community in order to build service user confidence and independence to achieve what matters to them.

The team is made up of Physiotherapists, Speech & Language Therapists, Dietitians, Nurses, Occupational Therapists, Rehabilitation Assistants, Doctors, Age Connects brokers and Social Services staff. They work closely with the local acute hospital teams and GPs to provide timely discharge of patients from hospital and support at home to reduce unnecessary hospital admissions where this is appropriate.

The successful applicant will need to demonstrate an interest in rehabilitation and excellent communication skills are key. It is important that you are a team player with a positive outlook, enthusiasm and ability to learn and develop new skills. Previous experience in rehabilitation would be advantageous.

Sound IT skills will be required and you will be will be provided with any equipment required to support agile working. The post holder may be required to work in other areas depending on service needs.

Main duties of the job

To provide a service with the Therapists, which will assist in the provision of flexible, patient centred rehabilitation programmes for patients. To work, under supervision, to assist the Therapists in clinical and administrative duties, following specific competency- based training. To be delegated routine treatment programmes to be carried out independently, and progress treatments within guidelines laid down by the Therapists. To be responsible and accountable for these tasks. The post holder may be required to work in other areas depending on service needs.

The post is community based and therefore the ability to travel to meet the demands of the post in a timely and efficient manner is essential. This is a community post which requires the post holder to travel to a number of patient's homes each day, often carrying large equipment such as clean toilet frames, kitchen trolleys and Zimmer frames.
