Job summary

An exciting opportunity has arisen for an enthusiastic and motivated nurse to join the expanding Supportive Care Team within Cardiff and Vale. Theaward-winningSupportive Careservice is a relatively new service development, whichhassignificantly improve patient care and experienceinadvanced heart failure, liver, renal and respiratory and is now expanding into other life-limiting non-malignantdisease areas.

This is a development role within the Supportive Care team and you will be supported to develop skills and your education around the clinical assessment and management of symptoms for patients within the hospital and community setting.

Our range of patients require diverse skills and thus in joining the team you will develop new skills and knowledge as you bringing your existing skills and knowledge to share with us.

You will be working closely with and be supported by the wider multidisciplinary Supportive Care team of clinicians, clinical nurse specialists and therapies. You will have the opportunity to contribute to education, research and service development to improve the care provided to our patients in their last 1-2 years of life.

You should have excellent interpersonal and communication skills as this role will include difficult conversations with patients and families and involve communicating with a variety of hospital and community teams across Cardiff and Vale UHB and other agencies.

Main duties of the job

  • The aim of the role is tocontinue to developand deliver a seamless pathway of patient care from point ofreferral to discharge from service.The overarching aim is to improve patient experiencein a value-based mannertoprovide and support patientcarethroughout their journey right up to thefinal phase of life.

    You will be part oftheSupportiveCare Teamensuring clear communication and documentation is completed in keeping with Health Board Policy and adhering to confidentiality.

    The successful candidate willbe requiredto promote effectiveness within the team bydemonstratingHealth Board values and a positive and inclusive working culture to support staff well-being, whilstdemonstratingand promoting professional curiosity.

    To develop a management plan in regards to appropriate symptom management or support that can be offered.

    To evaluate care and treatment outcomes.To provide clinical advice to all members of the multidisciplinary team, patients, carers and other agencies.
