Job summary
We are looking for a nurse with a background in acute and/ or rehabilitation care and with a specialist interest and knowledge of stroke. Candidates need to demonstrate an enthusiasm and commitment to delivering and developing the Stroke service across the Health Board.
The Early supported discharge (ESD) Nurse plays a unique contribution within the ESD team. The ESDT team is made up of physiotherapists, occupational therapists, speech and language therapist, psychology and nursing. The nursing role will involve visiting patients within their own homes to ensure they continue receiving nursing care whilst receiving stroke rehabilitation in the community. Common problems after stroke reported by patients at home, contain a wide range of nursing needs including, blood pressure monitoring, pain assessment, managing mood and fatigue, medication administration and bladder and bowel care. Further, secondary stroke prevention is a key aspect of the role, by ensuring patients receive education on their stroke diagnosis and how to reduce their future risk.
The post holder will provide education and support to people who have suffered from a stroke as well supporting other healthcare professionals, whilst being actively engaged in service improvement initiatives. You will be required to work autonomously but will be fully supported to develop within your role as a specialist nurse, while working closely with members of the Multi Disciplinary Team.
Main duties of the job
The successful candidate will work alongside the ESD Team to in reach to patients on the acute stroke ward at UHW and SRC, identifying patients who are suitable candidates for the ESD service.
The post holder will be responsible for manging their own case load of patients and ensuring they receive on going stroke nursing care in the community. The ability to be able to transfer between patients' homes is essential. The post holder will attend multidisciplinary meetings to discuss patients progress and identify any issues that need to be escalated. The post holder will be responsible for raising concerns with the patients GP or Stroke Consultant. Part of the role involves following up stroke patients at 6-months post stroke diagnosis, this can involve sign posting patients to various organisations that can support their needs.
The ability to transport between various sites and to home visits is essential for this role. A basic knowledge of using IT is desirable as cases notes are recorded via online system. The post will become familiar with the Sentinel Stroke National Audit Programme (SSNAP) quality improvement programme.
The post holder will provide specialist educational support to a range of healthcare professionals, ensuring the most up to date stroke guidelines are followed.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
