Job summary
The post holder will provide support for young people with diabetes across the Cardiff & Vale UHB catchment area. A particular focus of the post will be the transition period from paediatric to adult care and will cover the age group of approx 11-25 years.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job
To deliver and contribute to the development of recreational, social and general support services for young people with diabetes cared for by Cardiff and Vale UHB.
To provide one-to-one support to young with diabetes people referred to the Service who have been identified as having specific needs
To monitor, prioritise and manage referrals received from both Paediatric and Young Adult diabetes services in Cardiff and Vale UHB.
To work flexibly with the diabetes teams to provide a varied programme of activities and interests, recreational and social facilities during the day and evening, with due regard to the needs and opinions of young people.
To participate as a professional member of the Diabetes clinics and contribute to the care of the patients as part of the multidisciplinary team relating to their all round cultural, development and emotional needs.
To work collaboratively with other transitional teams within the Health Board
To contribute to the development of a Clinical Governance framework within the service.
The post is full time, 37.5 hours per week, although requests to work on a part time basis will be considered. Hours of work to be flexible to meet the needs of the client group, to include some evenings and weekend activity.
The appointee will receive supervision from a trained clinical psychologist with expertise in paediatric diabetes.
