Diabetes Youth Worker

Job summary

The post holder will provide support for young people with diabetes across the Cardiff & Vale UHB catchment area. A particular focus of the post will be the transition period from paediatric to adult care and will cover the age group of approx 11-25 years.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Main duties of the job

To deliver and contribute to the development of recreational, social and general support services for young people with diabetes cared for by Cardiff and Vale UHB.

To provide one-to-one support to young with diabetes people referred to the Service who have been identified as having specific needs

To monitor, prioritise and manage referrals received from both Paediatric and Young Adult diabetes services in Cardiff and Vale UHB.

To work flexibly with the diabetes teams to provide a varied programme of activities and interests, recreational and social facilities during the day and evening, with due regard to the needs and opinions of young people.

To participate as a professional member of the Diabetes clinics and contribute to the care of the patients as part of the multidisciplinary team relating to their all round cultural, development and emotional needs.

To work collaboratively with other transitional teams within the Health Board

To contribute to the development of a Clinical Governance framework within the service.

The post is full time, 37.5 hours per week, although requests to work on a part time basis will be considered. Hours of work to be flexible to meet the needs of the client group, to include some evenings and weekend activity.

The appointee will receive supervision from a trained clinical psychologist with expertise in paediatric diabetes.

1. Ensure that the environment is conducive in meeting the needs of the young person and family including the safety, care and maintenance of equipment required.

2. To act in accordance with the National Youth Agency Ethical and Professional Code.

3. To be flexible and able to adapt when responding to referrals in a variety of clinical areas.

4. To engage fully with the supervision provided by the clinical psychologist and clinical paediatric and adult diabetes teams.

Clinical Duties

1. To provide a high standard of holistic care to young people and their families, taking into account their individual needs.

2. To support staff involved in the care of young people regarding the needs of young people within the health care environment.

3. Undertake assessments and complete records in line with departmental and Health Board policies at Cardiff and Vale UHB, liaising with others as appropriate.

4. Assess the social, developmental and educational/vocational needs of young people and groups of young people and introduce appropriate programmes or activities.

5. Act as a key worker to whom young people and health care workers can refer for support and advice.

6. Establish relationships with young people and as appropriate with parents /carers and siblings.

7. Offer advice and information to young people and their families relating to psychosocial, educational/vocational issues and generic health issues such as substance use and sexual health.

8. Create opportunity for young people to participate in service development and actively share their views across the Health Board.

9. Work closely with health care professionals in the development of adolescent and young peoples clinics.

10. Encourage young people to self-advocate or advocate on their behalf, working closely with C&V UHB.

11. Document and report any concerns relating to needs assessment and child protection issues as appropriate.

12. Participate in multi disciplinary meetings, case conferences, reviews etc providing professional advice on individual care programme planning, reporting any relevant information or concerns as necessary, making referrals and maintaining appropriate links.

13. Work closely with other health care professionals offering advice and support to young people and their families through the process of transition and transferring into adult care.

14. Support young people in their home / community, if necessary, to help re-establish their normal routines e.g. returning to education or work. Enable young people to access appropriate community support services such as health, social, educational or legal services. Establish contact and relationships with groups and organisations to help establish young people in their communities.

5. Record appropriate information and follow identified reporting mechanisms.

6. Seek links with other hospital based youth workers.

STAFF TRAINING AND DEVELOPMENT

1. Undertake teaching sessions for other professional groups as required.

2. Supervise student placements in the departments as required.

3. Present outcomes of work at study days and conferences.
