Job summary
An exciting opportunity had arisen for an enthusiastic and dynamic pharmacist to join the Primary, Community and Intermediate Care Medicines Management Team, within the Medicines Optimisation workstream.
With your knowledge of national and local guidance you will respond to specific queries from primary care contractors regarding medicines optimisation. You will be involved in developing the medicines optimisation work-programme in primary care as well as providing high quality evidence based prescribing advice.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job

Provides patient-orientated prescribing support to primary care contractors (e.g. General Practices, community pharmacies, dentists and optometrists) and community teams (including care homes)

Provides specialist pharmaceutical advice and support to improve patient safety, cost-effective prescribing and clinical outcomes.

Provides support and supervision to other pharmacists, pharmacy technicians and support staff.

Communicates and works effectively across all areas of primary and community care to deliver a clinical pharmacy service to the whole health community.
