Children's Community Nurse with Integrated Community Nursing Service

The closing date is 04 December 2025

Job summary

Children's Community Nurse - ICCNS

We're seeking a motivated, compassionate Band 5 Nurse to join our Integrated Children's Community Nursing Service (ICCNS), a dynamic team delivering high-quality, family-centred care across Cardiff and the Vale of Glamorgan.

Open to Registered Children's Nurses, Learning Disability Nurses, or Adult Nurses with relevant experience or transferable skills in community-based paediatric care.

You'll provide nursing care to children and young people with acute, chronic, complex, and palliative needs in homes and community settings, working closely with families and multidisciplinary teams. You'll also support service development, including our Weekend Club and future respite facility plans.

We're passionate about making a meaningful difference. If you share our commitment to compassionate care, innovation, and excellence, we'd love to hear from you.

The successful candidate will be supported to undertake the Autonomous Practice module to enhance clinical decision-making and independent practice.

We offer flexible working, including part-time contracts from 20 hours and annualised hours, supporting work-life balance while meeting service needs.

Main duties of the job

As part of the Integrated Children's Community Nursing Service (ICCNS), you will have the opportunity to make a meaningful difference to families who receive Continuing Care (respite) from our dedicated and compassionate team.

The successful candidate will be able to demonstrate a broad range of clinical, interpersonal, and professional skills essential for this role, which involves both autonomous practice and collaborative team working. In return, we offer excellent training and development opportunities, along with the opportunity to support other elements of the service --including mainstream and special schools--providing variety and job satisfaction.

Care is delivered across a range of shifts and settings, including weekdays, week-nights, and weekends. Therefore, flexibility in working patterns is essential. The team provides care to children and young people with very complex and additional health needs, including those with tracheostomies and those requiring long-term home ventilation.

Key responsibilities include:

  • Demonstrating, teaching, and supervising nursing care that families can safely undertake, enabling them to carry out recommended treatments confidently and effectively.
  • Delivering a high standard of nursing care to patients in their own homes or other places of residence.
  • Actively participating in the learning and development of allocated students and newly appointed staff.
