Job summary
An exciting opportunity has arisen to join un as a Band 7 Discharge Liaison Nurse (DLN) , working within the Patient Flow Team in the Mental Health Services for Older People (MHSOP) directorate. There are two posts in total.
The postholders will work alongside the team, which is made up of Social Workers, Social Work Assistants and a Nurse Team Lead, primarily based in the MHSOP corridor, University Hospital Llandough. They will work in close partnership with Clinical Leads, Social Services and Third Sector and Voluntary Sector.
In addition the DLN's will receive support and supervision from managers and colleagues in the Complex Care and Commissioning Team. There will be an element of agile working, including carrying out placement reviews
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
Main duties of the role will include:
- Responsible for providing professional and clinical leadership in the nursing management of patients requiring complex discharge planning.
- Responsible for the operational management of NHS Continuing Health Care.
- Responsible for the operational management of NHS Funded Nursing Care on behalf of the Health Board.
- Provide a key advisory role to the multi-disciplinary team on discharge planning within the designated areas of responsibility.
