Job summary
A fantastic opportunity has arisen for a proactive and motivated Dietitian to be our lead dietitian for Community Neuro-rehabilitation in Cardiff and Vale UHB, based in both Llandough Hospital and Riverside Health Centre.
This post designs and delivers the dietetic service to the Early Supported Discharge (ESD) multidisciplinary team for stroke patients. In addition, the diverse workload includes a variety of complex neuro-rehabilitation patients, managed via individual home visits and group education.
In addition, the post holder will provide supervision and support for a Band 6 dietitian delivering "Live Well" services.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The Band 7 Neuro-Rehabilitation dietitian will develop, manage and co-ordinate a community dietetic service for the specialist client group.
This will involve working in partnership with other professions and agencies in a variety of settings including community venues, as well as directly with clients at an individual and group level. You will be working predominantly with a population with diverse and complex neurological health and social needs.
The post holder is the lead clinician for:
The management of overall dietetic service to this specialist area including development, implementation and evaluation. Working in partnership with key stakeholders to enable delivery of the service objectives within the available resource
the design, delivery and evaluation of nutrition and dietetic component of group education courses that are evidence based, using education theories and to an appropriate level for the target audience
Production of regular monitoring and evaluation reports for Health Board and partner organisations, on service delivery
Supervision and training of Project Support Workers and other workers involved in the specialist area, including community support workers, junior dietitians and students
To develop and co-ordinate the dietetic service for this area, undertaking research and audit to enable you to fully evaluate the continual effectiveness of the service
